Before our team can begin working on your EMMA implementation, we need some essential information from your practice. This information is collected through a short onboarding form that you’ll need to complete.
The details provided in this form are used directly to configure EMMA for your practice, so it’s important that all sections are completed fully and accurately. Missing or incorrect information can lead to delays or configuration issues later in the implementation.
We recommend setting aside some dedicated time to complete the form and ensuring you have the relevant practice details available before you start.
The information you provide allows us to:
- Design your call flows and patient journeys
- Configure system and telephony integrations
- Set up opening hours, call handling times, and failover routes
- Ensure the right contacts are in place for clinical, operational, and technical support
Without this information, we’re unable to progress with configuration or move toward go-live.
What the Form Covers
The form asks for practical details you should already have access to, including:
- Practice details (ODS code, address, contact numbers)
- Opening hours and phone handling preferences
- Key contacts within your practice and PCN
- Core clinical and administrative systems you use
- Telephony setup and provider details
- High-level staffing information
You are not expected to provide anything technical or create detailed workflows. The questions are structured to guide you clearly through what’s needed.

The form typically takes 20–30 minutes to complete.
